The problem is that you add another field "EMAIL" but in the INSERT section you did not make the change... so, it's best to upload the file as it and make the change in the VNSB_table thru your phpmyadmin.

OR
you can dowmload the zip file again from my site (NOT from sourceforge.net)


I just included the new VNSB_database.sql with "EMAIL" already included.
